5. Sign-in sheet. Your name may be called or written on a sign-in sheet.

8. Sale of health information. We do not sell PHI.

22. Psychotherapy notes. We will not use or disclose psychotherapy notes without prior written authorization, except for treatment, training, legal defense, or as required by law.
23. Research. PHI may be disclosed to researchers when allowed by law.
24. Fundraising. We do not engage in fundraising.

Patients are asked to acknowledge receipt of this notice at their first visit. The acknowledgment reads:
